Does L-arginine work for erectile dysfunction?
At gram doses, some trials say it did better than placebo in men with mild to moderate ED. The effect is modest, and it does not apply to small doses.
A 2019 review pooled 10 randomized trials with 540 men with mild to moderate ED. Doses were 1,500 to 5,000 mg a day. The odds of improvement were 3.37 times those of placebo or no treatment, and sexual desire scores did not change (Rhim 2019, Arginine for ED (meta-analysis)). Side effects were reported by 8.3% on arginine and 2.3% on placebo, none severe.
The authors noted that dose and duration varied. All the men had diagnosed ED, not healthy men.
The 2024 review
What did the 2024 review of supplements for ED find?
It found little. That review pooled 15 placebo-controlled trials with 1,000 men and said no regimen other than prescription combinations showed statistically significant efficacy (Barbonetti 2024, Nutraceuticals for ED (network meta-analysis)).
L-arginine alone ranked fifth. The authors called it somewhat useful, against a background of general ineffectiveness of most nutraceuticals, especially alongside prescription ED drugs in men with physical causes.
